Practical driving test application form
The driving test application form allows you to book a practical driving test. For internet or phone bookings, you do not need to fill in this form.
You can pay for the driving test by credit or debit card, by cheque or postal order. You can fax or send the completed application form by post. If you have not received any booking confirmation in two weeks you should phone the booking line number.
Although this is the most up to date form, the 04/05 version is in circulation and can still be used.
